SIBO, or small intestinal bacterial overgrowth, may be an important and frequently overlooked piece of the chronic gut health puzzle. In this episode of Reverse Crohn’s and Colitis Naturally, Josh Dech explores how excessive microbial growth in the small intestine can contribute to bloating, abdominal pain, altered bowel movements, malabsorption, and intestinal dysfunction.
The discussion breaks down what makes the small intestine different from the colon and why an abnormal microbial population in this area can create problems through premature fermentation and the production of gases and other metabolic byproducts.
Josh also explores the potential root causes behind SIBO, including impaired stomach acid, bile flow, migrating motor complex dysfunction, ileocecal valve issues, reduced immune defenses, antibiotic exposure, parasites, mold exposure, chronic stress, certain medications, and other underlying conditions.
A particularly important part of the conversation is the relationship between food poisoning, post-infectious IBS, and gut motility. The episode examines the proposed role of CdtB and vinculin antibodies and molecular mimicry in damage to the migrating motor complex—one possible mechanism behind recurrent SIBO.
You’ll also learn about the major SIBO/intestinal methanogen overgrowth patterns: hydrogen, methane, hydrogen sulfide, and mixed presentations. The episode covers breath testing, antibody testing, and small-intestinal aspirate culture, as well as the different therapeutic strategies commonly discussed for each pattern.
Most importantly, this conversation looks beyond simply “killing the bacteria.” With SIBO frequently recurring, addressing motility and the underlying conditions that allowed the overgrowth to develop may be an essential part of a root-cause approach to gut health.
If you’re struggling with persistent digestive symptoms, inflammation, or unexplained changes in bowel function, this episode provides a framework for understanding how the microbiome, immune system, gut motility, and root causes can intersect.
